New Patient Information

Pre-registration can be completed on our University Foot Associates Patient Portal. New patients can request access to their patient portal when booking an appointment by providing their email address, and will receive an emailed link to register.
Alternatively, please arrive for your New Patient appointment at least 10 minutes prior to your appointment time, to complete registration in the office.
